61 Y/M with multiple myeloma

61-year-old male, a mason by occupation presented to the casualty with  complaints of fever for 10 days  cough since 1week  shortness of breath since 1week   HISTORY OF PRESENT ILLNESS: The patient was apparently asymptomatic 3 years back then he developed Involuntary movements of the right upper limb which was initially less intense then slowly progressed to the rest with an increase in amplitude and frequency,  not associated with other abnormalities like weakness of limbs, giddiness, syncope.  associated with slowing of movements and difficulty in turning while walking for the past 2years for which he even stopped working. the patient went to a local practitioner for the same and was given medications but was in vain, he even traveled all the way to Karnataka to get his condition treated with local medication but was found to have no improvement in his symptoms . 2 years back patient started to increase his alcohol consumption after his wife demise and was having these mood distur

70 /M with pedal edema

70 year old male farmer by occupation came with complaints of pain in bilateral lower limbs and edema since 6 months  Neck pain radiating to hands since 1 month  Patient was apparently asymptomatic 20 years back then he developed pain and generalised weakness of body for which he stopped working as farmer and started to stay near his farm animals as rancher and with almost sedentary life style  3 years back patient developed pain in multiple joints increased with movements so went for local practioner and was taking occasional pain killers (?nsaids) once every 2 months  6 months back patient started to develop swelling of both lower limbs from the level of thighs and associated with pain in the lower limbs, oliguria since 1 month then came to our hospital and was investigated  Found to have  Serum creat :4.5 Urea:152  K : 5.3 Usg : Bilateral grade 2 rpd changes , lt simple Cortical cysts Patient was started on Tab Lasix  Pedal edema subsided and is grade 2 pitting type  Not associated

HOPI: patient was apparently asymptomatic 10 days back then he had pain in the right loin region which is insidious in onset and gradually progressive not associated with nausea vomiting and loose stools.
